Thanks for choosing Spectrum for your eye care needs! We want to make your visit as safe and efficient as possible so we ask that you please fill out these forms beforehand. This will allow us to focus on your eye care needs instead of paperwork when you are in office.
Please note: we require that you complete both steps no less then 48 hours prior to your appointment. If your forms are not received 48 hours before your appointment, unfortunately your appointment will be removed from our schedule.

Please note: as mandated by the College of Optometrists of Ontario, face masks are required by everyone entering the office.

Please cancel your appointment if you have any of the following symptoms or have been around anyone with these symptoms: coughing, fever, shortness of breath, muscle aches, significantly more tired than normal, sore throat, loss of taste or smell, chills, nausea or diarrhea.

Lastly, please cancel your appointment if you have travelled outside of Canada or have been around someone who has travelled in the last 14 days.

Please note:  To gain access into the mall for your appointment go to the dental office entrance on the upper level of the mall.  Please call security at 905-630-1058 to gain access if a security guard is not stationed at the door.
